Postresection urinary tumor DNA sharpens residual disease and recurrence prediction in NMIBC
Retrospective · JCO Precis Oncol · 2026-11-06. Number 1 in the UrologyWatch Top 5 for November 7, 2026.
In 161 NMIBC patients undergoing repeat TURBT, residual tumor was found in 57 (35.4%); adding utDNA to core, expanded, and CIS-augmented indication sets raised AUC for residual tumor detection from 0.544, 0.537, and 0.513 to 0.891, 0.892, and 0.890 respectively, and improved recurrence c-indices from 0.602 to 0.774 (EAU), 0.717 to 0.837 (EORTC), and 0.770 to 0.824 (CUETO).
Why it matters: Suggests utDNA could help refine selection for repeat TURBT and recurrence risk stratification beyond current guideline-based models, pending prospective validation.
Limitation: Guideline-anchored, single-institution cohort study without independent validation or prospective testing of clinical decision impact.
