Non-BRCA2 HRR gene alterations show divergent survival in mCRPC cohort
Prospective · Eur Urol Oncol · 2026-11-04. Number 1 in the UrologyWatch Top 5 for November 4, 2026.
In a prospective cohort of 1010 men treated with first-line therapy for metastatic castration-resistant prostate cancer, ATM and CHEK2 alterations were associated with better overall survival than BRCA2, while BRCA1, FANCA, and CDK12 alterations showed survival similar to BRCA2 and worse than no HRR alteration.
Why it matters: Suggests individual HRR genes beyond BRCA2 carry different prognostic weight, a consideration for interpreting panel testing results.
Limitation: Prospective cohort with multivariable adjustment but not randomized; causal inference about treatment selection is limited.
