Hematuria evaluation is infrequent and shows gender and race disparities in national cohort

Prospective · J Urol · 2026-11-04. Number 3 in the UrologyWatch Top 5 for November 4, 2026.

In a prospective All of Us cohort of over 16,000 patients with hematuria, only 43% were ever evaluated with cystoscopy or imaging over 6+ years, with women having roughly half the cystoscopy rate of men and some racial differences by hematuria severity.

Why it matters: Raises the question of systemic gaps and inequities in guideline-concordant hematuria workup that may warrant closer attention to referral pathways.

Limitation: Observational cohort from a research database; cannot fully capture clinical reasoning behind decisions not to pursue evaluation.
