Urine tumor DNA after induction therapy predicts who benefits from maintenance IVT

Prospective · Eur Urol Oncol · 2026-11-02. Number 3 in the UrologyWatch Top 5 for November 2, 2026.

In a 140-patient prospective multicenter study (INSITE), utDNA-positive patients had much better recurrence-free survival with maintenance intravesical therapy than without (68% vs 16% at 24 months), while utDNA-negative patients showed no significant benefit from maintenance.

Why it matters: Adds support for using postinduction utDNA to help identify which NMIBC patients are likely to benefit from maintenance IVT versus candidates for de-escalation.

Limitation: Observational study with nonrandomized treatment assignment; biomarker-guided de-escalation not yet tested prospectively.
