Bone health agents tied to lower fracture odds with radium-223 plus enzalutamide; prior abiraterone not linked
Retrospective · Prostate Cancer Prostatic Dis · 2026-11-02. Number 4 in the UrologyWatch Top 5 for November 2, 2026.
In a retrospective cohort of 171 men with mCRPC treated with radium-223 plus enzalutamide, 15% developed fractures. Prior abiraterone exposure was not associated with fracture risk (OR 1.00). Concurrent bone health agent use was associated with 79% lower odds of fracture (OR 0.21, 95% CI 0.07-0.57, p=0.010), a relative, not absolute, reduction.
Why it matters: Raises the question of whether bone health agent use during radium-223 combinations warrants closer attention, though this retrospective, association-based analysis cannot establish causation.
Limitation: Retrospective claims database analysis without randomization; residual confounding by indication likely.
