Combining PSMA PET with MRI improves focal therapy eligibility detection versus either alone
Retrospective · J Nucl Med · 2026-10-30. Number 2 in the UrologyWatch Top 5 for November 1, 2026.
In 112 men validated against whole-mount prostatectomy specimens, combining PSMA PET/CT and MRI increased sensitivity to detect true ineligibility for focal therapy to 83% compared with each test alone, but specificity fell to 26%, meaning more eligible patients would be incorrectly excluded.
Why it matters: Suggests dual imaging could better flag disease extent unsuitable for focal therapy, but the trade-off is many false positives that would exclude otherwise appropriate candidates.
Limitation: Retrospective single-center study with modest sample size and substantial loss of specificity when combining modalities.
