Combined nodal status and lymphovascular invasion refine risk stratification after radical cystectomy
Retrospective · Eur Urol Focus · 2026-10-30. Number 4 in the UrologyWatch Top 5 for November 1, 2026.
In a multicenter cohort of 1,229 patients after upfront radical cystectomy, lymphovascular invasion was independently associated with worse disease-specific survival in both node-negative and node-positive disease, creating three prognostic groups with 5-year disease-specific survival of 65%, 48%, and 25%.
Why it matters: Raises the question of whether lymphovascular invasion status could help refine postoperative counseling and adjuvant therapy discussions, pending prospective validation.
Limitation: Retrospective multicenter cohort spanning three decades; adjuvant chemotherapy receipt was not randomized.
