Fluciclovine PET finds disease in over half of recurrences missed by PSMA PET

In a prospective single-arm study of 50 men with biochemical recurrence and negative PSMA PET, fluciclovine PET/CT was positive in 56% of patients, with detection rising with PSA level, and changed intended management in 71% overall.

Why it matters: Suggests fluciclovine PET could have a complementary role for localizing recurrence when PSMA PET is negative, though this needs validation against pathologic confirmation in larger cohorts.

Limitation: Single-arm, single-center study of only 50 patients with reference-standard confirmation available in just 14 cases.
