Patterns of imaging progression on Lu-177-PSMA therapy mapped in 361-patient cohort

Retrospective · J Nucl Med · 2026-10-30. Number 5 in the UrologyWatch Top 5 for October 30, 2026.

In a two-center retrospective review of 361 men on at least two cycles of Lu-177-PSMA therapy, 80% showed shrinking or fewer lesions on serial imaging while 43% eventually progressed, often with new PSMA low-avidity bone lesions.

Why it matters: Raises the question of how posttreatment imaging patterns, including low-avidity or PSMA-negative progression, could inform monitoring during PSMA radioligand therapy.

Limitation: Retrospective, two-center cohort without prespecified imaging response criteria validated against outcomes.
