POTOMAC: durvalumab plus BCG improves DFS in papillary-only, BCG-naive high-risk NMIBC

Phase 3 RCT · OncLive · 2026-10-24. Number 2 in the UrologyWatch Top 5 for October 28, 2026.

In a prespecified subgroup of 437 patients with papillary-only high-risk NMIBC (median follow-up 60.7 months), durvalumab with BCG induction and maintenance reduced disease-free survival events versus BCG alone (HR 0.56, 95% CI 0.37 to 0.84; 17% vs 30% with events). DFS at 48 months was 84% vs 74%. In the post hoc T1 subset, the HR was 0.48.

Why it matters: Extends the POTOMAC benefit to papillary-only disease, the largest group of BCG-naive high-risk patients.

Limitation: Subgroup analysis; median DFS not reached in either arm.
