PEACE-1 long-term update: abiraterone added to ADT plus docetaxel extends survival in de novo mCSPC
Phase 3 RCT · ecancer · 2026-10-24. Number 1 in the UrologyWatch Top 5 for October 28, 2026.
At a median follow-up of 7.7 years, in the 710 men treated with ADT plus docetaxel, adding abiraterone improved overall survival (HR 0.68, 95% CI 0.56 to 0.82; median 6.2 vs 4.4 years). In high-volume disease, median OS was 4.6 vs 3.5 years (HR 0.68) and 38% vs 20% were alive at 8 years. In low-volume disease, median OS was 7.8 vs 6.2 years (HR 0.74, 95% CI 0.52 to 1.05), not significant on its own.
Why it matters: Strengthens the case for triplet therapy (ADT, docetaxel and abiraterone) in de novo high-volume metastatic castration-sensitive prostate cancer.
Limitation: Results from a meeting presentation and press release; volume subgroups are smaller.
