Initial enfortumab vedotin dose reduction not clearly linked to worse survival in pretreated urothelial cancer

Retrospective · JAMA Netw Open · 2026-10-23. Number 4 in the UrologyWatch Top 5 for October 23, 2026.

In a multicenter Korean cohort of 368 patients with pretreated advanced urothelial carcinoma, physician-selected initial dose reduction of enfortumab vedotin showed no statistically significant difference in progression-free or overall survival compared with full dose, though confidence intervals were wide.

Why it matters: Raises the question of whether upfront dose reduction can preserve outcomes in selected patients, but this retrospective comparison cannot establish equivalence.

Limitation: Retrospective multicenter cohort with potential confounding by indication; wide confidence intervals around hazard ratios.
