About 1 in 4 men with suspicious MRI and negative biopsy later found to have significant cancer

Retrospective · Prostate · 2026-10-22. Number 3 in the UrologyWatch Top 5 for October 22, 2026.

In a post hoc subgroup analysis of 153 men from the MULTIPROS trial with PI-RADS ≥3 lesions and an initial biopsy negative for clinically significant cancer, csPCa was later detected in 24.2%, three-quarters within 6 months.

Why it matters: Hypothesis generating only: early clustering of detections suggests possible initial sampling misses, but repeat-biopsy rates differed between groups making comparisons hard to interpret.

Limitation: Post hoc, non-protocol-mandated follow-up with differential repeat-biopsy rates and low sensitivity of the predictive model at standard thresholds.
