PSMA PET/MRI outperforms MRI-targeted biopsy for equivocal PI-RADS 3 lesions

Prospective · J Urol · 2026-10-09. Number 1 in the UrologyWatch Top 5 for October 9, 2026.

In a single-center prospective study of 66 men with equivocal PI-RADS 3 lesions, PSMA PET-targeted biopsy detected clinically significant cancer in 15% vs 6% with MRI-targeted biopsy alone, a 9 percentage point difference.

Why it matters: PSMA PET added to equivocal PI-RADS 3 lesions could reduce unnecessary biopsies while catching more significant cancers.
