Umbrella review finds low-quality evidence behind intracavernosal BoNT-A for refractory ED

Meta-analysis · J Sex Med · 2026-10-08. Number 4 in the UrologyWatch Top 5 for October 8, 2026.

An umbrella review of 4 systematic reviews covering 10 unique primary studies (n=865) on intracavernosal botulinum toxin for erectile dysfunction found very high study overlap (CCA 53.3%), critically low to low AMSTAR-2 ratings, high risk of bias by ROBIS, and low GRADE certainty, despite consistently reported improvements in erectile function.

Why it matters: Highlights that existing systematic review evidence for this off-label ED therapy is methodologically weak, warranting caution before wider adoption.
