Testosterone breakthrough during ADT intensification linked to worse outcomes in LATITUDE, not ARASENS

Retrospective · BJU Int · 2026-10-06. Number 4 in the UrologyWatch Top 5 for October 6, 2026.

Post hoc landmark analyses of two phase 3 mHSPC trials found testosterone breakthrough above 35 ng/dL was associated with worse overall survival in LATITUDE (HR 1.49) but not significantly in ARASENS (HR 1.18 for OS, HR 0.86 for time to CRPC).

Why it matters: Raises the question of whether monitoring and correcting testosterone breakthrough during ADT intensification could improve outcomes, though trial-specific differences warrant caution.
