PROpel subgroup analysis: olaparib-abiraterone shows modest rPFS gain without BRCA mutation

Retrospective · Eur Urol Oncol · 2026-10-06. Number 1 in the UrologyWatch Top 5 for October 6, 2026.

In a post hoc subgroup analysis of the Phase 3 PROpel RCT's non-BRCA mutated patients (n=693), olaparib plus abiraterone gave longer median rPFS than placebo plus abiraterone (24.1 vs 19.0 months, HR 0.76, 95% CI 0.61-0.94), but overall survival was not significantly different (39.6 vs 38.0 months, HR 0.91, 95% CI 0.73-1.13).

Why it matters: Clarifies that the rPFS benefit of adding olaparib to abiraterone extends to non-BRCA mCRPC in this post hoc analysis, but survival benefit remains unproven in this subgroup.
