Gleason pattern 4 length under 2mm may expand active surveillance eligibility pool
Retrospective · BJU Int · 2026-10-06. Number 2 in the UrologyWatch Top 5 for October 6, 2026.
In a cohort of 2499 radical prostatectomy patients, those with less than 2mm of total Gleason pattern 4 length had adverse pathology and 5-year biochemical recurrence rates similar to favorable intermediate-risk disease, regardless of Grade Group, suggesting this threshold could roughly double the population eligible for active surveillance.
Why it matters: Suggests quantitative Gleason pattern 4 burden could refine active surveillance criteria beyond current risk grouping, pending validation.
