PSMA-guided radiotherapy intensification shows modest FFS benefit at 5 years

Phase 1-2 · Eur Urol · 2026-10-04. Number 1 in the UrologyWatch Top 5 for October 4, 2026.

In a phase 2 randomized trial (n=253), PSMA-PET-guided radiotherapy intensification to all detected sites did not significantly improve failure-free survival versus standard radiotherapy (66% vs 62% at 5 years), though freedom from biochemical progression favored the PSMA-guided arm.

Why it matters: Suggests PSMA-guided RT intensification may improve biochemical control but has not yet shown definitive failure-free survival benefit.
