The NCCN Bladder Cancer guidelines (current Version 3.2026) list the gemcitabine intravesical system (Inlexzo) as a category 2A option for BCG-unresponsive high-risk NMIBC with Ta/T1 papillary-only disease without CIS, in addition to its listing for CIS with or without papillary tumors.

Why it matters: Extends a bladder-sparing option to the papillary-only BCG-unresponsive population, which joins the other agents listed for this setting; payer coverage commonly follows NCCN listing.
